Applications for new certification or recertification can be sent electronically to nhpreventcert@gmail.com with the exception of the notarized Code of Ethics pages, which must be an original, notarized signature and mailed to NH Prevention Certification Board, 501 South Street, 2nd Floor, Bow, NH 03304. Payments can be sent by check or paid online via PayPal.
